A reminder to all parents of current 6th grade students. According to state regulations your child must have a tetanus booster (Tdap) before entering grade 7. Your child will not be allowed to start school in the fall without documentation of this vaccine. Immunization records can be mailed to the school over the summer or dropped off in the main office between the hours of 8am and 3pm. If you have any questions please feel free to call.
Also, starting the 2011-2012 school year all 7th grade students must provide a current physical exam to the health office. Physical forms are available to download on this site or can be sent home with your child by contacting me. Immunization requirements for those students entering 7th grade are as follows:
1 dose Tdap vaccine
2 doses varicella vaccine
2 doses MMR
